There are three current membership levels: $150 per year for Sustaining membership, $95 per year for Regular membership, and $35 per year for Student membership. In signing this form, you agree to send the appropriate amount by check, payable to the address at the top of the page.
Note: Your membership will not become effective until receipt of my check.
